Liability Waiver & Assumption of Risk

By signing below, I acknowledge and fully understand that participation in Arizona Soccer Skills Academy programs carries inherent risks, including but not limited to physical injury, illness, permanent disability, and death. I voluntarily assume all such risks and agree that Arizona Soccer Skills Academy, its owners, employees, agents, affiliates, and any associated facilities shall not be liable for any injuries, damages, or losses that occur before, during, or after participation, whether caused by negligence or otherwise.

I further agree to indemnify and hold harmless Arizona Soccer Skills Academy from any claims arising from my (or my child's) participation, including attorney fees and court costs incurred in defense of any claims made against the Academy.

Medical Authorization & Emergency Treatment

I certify that I (or my child) am in excellent physical health and may participate in strenuous and hazardous physical activities, including soccer. I acknowledge that Arizona Soccer Skills Academy does not provide medical services or emergency care on-site.

In the event of an emergency, I authorize Arizona Soccer Skills Academy and its representatives to obtain medical treatment for me (or my child). I understand that I am responsible for any and all medical expenses incurred. I confirm that I have appropriate insurance coverage for any injuries sustained during participation.

Media Release

I grant Arizona Soccer Skills Academy permission to use photographs or videos taken during the program for promotional purposes, without compensation. I understand that these materials may be used in print, online, or on social media.

Code of Conduct

Participants, parents, and spectators are expected to behave respectfully toward coaches, staff, and other participants. Disruptive behavior, inappropriate language, or failure to follow safety guidelines may result in removal from the program without refund.

Assumption of Weather & Environmental Risks

Outdoor sessions may be subject to extreme heat, cold, or other weather conditions. Participation is at the individual's own risk, and sessions may be modified or rescheduled due to weather without a refund.

Equipment Responsibility & Personal Belongings

Participants are responsible for their own equipment. Arizona Soccer Skills Academy is not liable for lost, stolen, or damaged personal items.

Injury & Illness Protocol

Participants who sustain injuries may be required to sit out until medically cleared. Parents must notify the Academy if their child has a contagious illness before attending sessions.

Parental Involvement & Supervision

For younger participants or participants who display behavior issues, parents may be required to remain on-site during sessions. Children must be picked up promptly after sessions. A late pickup fee may apply for extended supervision.

Cancellation & Refund Policy

1. No Refunds: 
Arizona Soccer Skills Academy does not offer refunds under any circumstances. Credits are issuesSee below:

2. Participant-Initiated Cancellations:
If a participant cancels a camp credit equal to the full amount paid will be issued. This credit is valid for one year and can be used toward any available camp or program.

3. Program-Initiated Cancellations:
Low Enrollment: If Arizona Soccer Skills Academy cancels a session due to low enrollment, participants will receive a credit equal to double the amount paid, valid for any future program.

Weather, Field Issues, or Other Unforeseen Circumstances: If a session is canceled due to weather conditions, facility issues, or other unforeseen circumstances, participants will receive a credit equal to the full amount paid, valid for any future program within one year.

4. Schedule Adjustments:

By registering, I acknowledge that camp schedules, locations, and programming may be subject to change. Arizona Soccer Skills Academy reserves the right to adjust programming as needed, and participants accept these changes without entitlement to a refund.

Payment & Registration Terms

All program fees must be paid in full at the time of registration. Late registration fees may apply. Credits issued for cancellations are non-transferable and cannot be converted into cash.

Dispute Resolution & Governing Law

Any disputes arising from participation in Arizona Soccer Skills Academy programs shall be resolved through arbitration rather than litigation. This agreement is governed by the laws of the State of Arizona.
